Athletic Participation Packets This folder includes valuable information required at the Mandatory Parent/Player - Athletic Director Meeting in August. Athletic Participation Packets are available for pick-up in the Athletic Director's office prior to the end of the school year. It includes the Athletic Director's letter and the following attachments: "Athletic Check List", "Voluntary Activities Participation Form", "History Form", "Physical Exam Form", and the "Physician Clearance, Personnel Data, Permission to Treat Form"
