EXAMPLE:

2004 CIF SCHOLAR-ATHLETE OF THE YEAR APPLICATION PACKET

=============================================

CIF State Office

333 Hegenberger Road, #511, Oakland, CA 94621 510/639-4445

=============================================

QUALIFICATIONS -- College-bound high school seniors whose academic and athletic careers have been truly exemplary and whose personal standards (i.e., integrity, honesty, etc.) and accomplishments are a positive model for others.

Students must:

1. Possess no less than an unweighted cumulative (grades 9-12) grade point average of 3.70;

2. Have demonstrated superior athletic ability in at least two years of varsity play within California (must be participating in athletics as a senior); and

3. Most of all have portrayed themselves as athletes of character displaying, trustworthiness, respect, responsibility, fairness, caring, and citizenship.

METHOD OF APPLICATION -- In past years we have limited schools to applications for one male and one female. At the schools' request, we will consider more than these applications from each school.

Each application must be accompanied by:

_ Student Application Forms (3), including signed verification of GPA by counselor or school site records clerk.

_ Letters of recommendation from principal or counselor and athletic director, as well as sportsmanship statement signed by coach.

_ Recent transcript of grades.

_ Student essay (see below).

STUDENT ESSAY -- To give us an idea of how well the students express themselves, we are asking that they write an essay about how they display character in their athletic and academic efforts. An applicant may wish to include: trustworthiness, respect, responsibility, fairness, caring, and citizenship. The essay should be written in the student's own words, with no outside help or influence. A maximum length of 500 words is recommended. The applicants might want to consider a few of the following concepts while formulating their thoughts:

_ Examples of meaningful behavior in your high school experience;

_ Lessons learned about the importance of character in your life;

_ Opportunities coaches, cheerleaders, athletes and fans have to promote character in interscholastic athletics.

Remember, the aforementioned are just suggestions. The students are encouraged to express themselves on what they value most about their sportsmanship experience and how it will affect their lives.

DEADLINES -- Students should deliver the completed applications to their counselor or principal by February 16, 2004, for review. It is the school's responsibility to notify students as to whether their packets were forwarded to the CIF State Office for further consideration. APPLICATIONS MUST BE POSTMARKED NO LATER THAN WEDNESDAY, MARCH 24, 2004. Late or incomplete applications will not be considered.

JUDGING -- Judging of applications will be conducted by committees selected by the CIF. Winner will be notified before MAY 17, 2004.

AWARDS

The two state winners (one male and one female) will receive:

_ $2,000 scholarship.

_ An expense-paid trip to our State Capitol in Sacramento to be presented to the legislature.

Actual presentation of all scholarship awards will be made at an event to be scheduled in the Spring. Recipients must be present at this event.

Every student meeting the requirements of the application and sending a completed application will receive a certificate suitable for framing.
