Physicals

A Parent/Guardian MUST SIGN the following forms

prior to the physical:

Physical Examination and

Physician Clearance, Personnel Data, Permission to Treat

Please bring all forms in the Athletic Participation Pack to the Physical

and the MANDATORY Informational Meeting.

by appointment only.
